There are many situations in which our at home medical and home aide services would benefit you such as:

All At Home Health Care professionals work together with your physician to provide a total home health care plan.

The first step is to request a referral for care from your physician or hospital discharge planner.

Once we receive your doctor's request, our referral coordinator will initiate a package of services, based on your current needs and the orders of your physician. 

Contact between All At Home staff and your physician is ongoing and our team will provide feedback to your doctor about your recovery progress. 
Our certified nurses are central to your care at home. They conduct patient and environment assessments and coordinate care with all team members. They also consult with your physician routinely and provide updates on your progress.  In addition to providing needed treatment, our nurses also educate patients and other caregivers on the disease or injury process, treatment plan, medication, diet, injections, IV infusions and wound care as needed.
Physical therapists work with patients to help improve strength and mobility. They also assess the patients' ability to move safely within the home and provide recommendations for a safe environment. Their main goal is to help the patient gain independence in walking, climbing stairs and other daily activities.

Occupational therapists develop exercise programs and training for personal care tasks such as bathing, preparing meals and fine motor coordination. They also provide instruction and adaptive equipment so the patient can improve their ability to care for themselves.    
Speech therapists work with patients to improve speech, memory and swallowing disorders and assist staff and family members to communicate effectively with the patient.
Home assistants are your helping hand around the house. From meal preparation, light house keeping, shopping, to personal care such as bathing and grooming they will take care of your chores so you can focus on getting well.

Our counselors help you work through emotional issues that can occur during an illness. They also assist with long range planning for future needs, and education on available community resources.
