Client Referral Checklist
A list of needed pertinent items for All At Home Health Care admission:

  • Current insurance information including company name, and any applicable ID or group numbers.

  • A physician’s order for home care services.  If the ordering physician is not the primary care physician please also provide the primary care physician’s name and telephone number.

  • Also make available the most recent history and physical information and/or progress notes that you have on file.

How to Get Started
All At Home Health Care professionals work together with your physician to provide a total home health care plan.

The first step is to request a referral for care from your physician or hospital discharge planner.

Call us at (480) 970- 3700 to help you with this process.

The second step: Once we receive your doctor's request, our referral coordinator, who is also a licensed Nurse, will set up an appointment with you to come to your home and initiate a package of services that is based on your needs and the orders from your physician.  We will also assign a Nurse or Therapist depending on the type of care you need and develop your treatment program and schedule of care.

Contact between All At Home medical professionals and your physician is ongoing. Our team will regularly report your recovery progress to your doctor.
